Level funded

You pay a fixed monthly amount that covers expected claims, stop-loss protection, and admin fees. If your team stays healthier than projected, you get money back. It's gaining traction with employers in the 25 to 200 range who want some upside without full self-funding risk.

Self-funded

You pay claims directly and carry stop-loss insurance to cap your downside. You see every dollar, control plan design completely, and keep the savings when claims are low. Best for employers with 75+ lives and a stomach for some variability.

PEO and association plans

Professional Employer Organizations and industry associations let small employers access large-group rates they'd never get on their own. If you have 5 to 25 employees and premium quotes make you wince, this is worth exploring.

HSA-qualified high deductible plans
